Team docs route from meeting decision to support handoff

A meeting decision note should let someone understand the choice weeks later without asking everyone to reconstruct the conversation.

The core mistake is saving a transcript or a bullet list of topics instead of the actual decision. People remember that a meeting happened, but not what changed after it. A good note says what was decided, who owns the next action, when the decision was made, and what context limited the options.

Use a compact structure:

1. Decision: one sentence naming the selected option.
2. Owner: the person or role responsible for follow-through.
3. Alternatives: the serious options that were rejected.
4. Reason: the constraint or evidence that made the chosen option better for now.
5. Risk: what might go wrong or what remains uncertain.
6. Review trigger: the condition that would make the team revisit the choice.

The review trigger is what keeps the note from becoming stale authority. If the choice depends on customer volume, support hours, budget, vendor pricing, or a deadline, write that condition explicitly. A future team can then say “the condition changed” instead of arguing from memory.

The note should also link to the source document, ticket, customer request, or policy that shaped the decision when that context is safe to share. Do not bury the conclusion under ten paragraphs. Put the decision first, then the reasoning.
