A handoff risk note names the thing most likely to fail after work moves from one person or shift to another. A handoff can look complete when it lists tasks but hides the fragile part. The fragile part may be an unconfirmed customer answer, a file that only one person can access, an invoice waiting for approval, a supplier deadline, a blocked account, or a step that depends on local knowledge. If this risk is not named, the next person may treat the work as routine. A good note separates status from risk. Status says what has happened. Risk says what could break next and what signal to watch. For example, “Quote sent” is status. “Risk: client asked for same-day delivery but stock is not confirmed; call warehouse before promising time” is a handoff risk note. The boundary is that the note should not become a fear list. Name the one or two risks that change the next person’s behavior. If every line is marked urgent, nothing is urgent. The note should help someone choose where to look first. The practical interpretation is: a handoff is better when the next person knows not only what is done, but what might quietly fail.
