A handoff note is a short operational record that lets another person continue work without needing the original person to explain the current state live. A good handoff note answers five questions: what changed, what is still open, what is blocked, where the relevant source is, and what the next person should do first. It should not try to preserve every detail. The goal is continuity, not a diary. The best format depends on the team. A support handoff may focus on customer state, severity, and pending replies. A product handoff may focus on decision history, open questions, owners, and links. A shift handoff may focus on incidents, checks already done, and unusual observations. A handoff note is different from a final report. A final report explains what happened after work is complete. A handoff note is written while work is still alive and uncertainty remains. It should show the next safe action, not just summarize the past. The practical test is simple: if the next person can start without asking “where is this?” or “what should I check first?”, the handoff note is doing its job.
