A Decision Action Ledger is a short meeting record that separates what was decided from what still needs to happen. It exists because many meetings produce notes but no accountable next step. A useful ledger has only a few fields: Decision: what changed after the meeting. Owner: one directly responsible person, not a team or department. Due date or check date: when progress will be reviewed. Open question: what is still unresolved. Reference: link to the agenda, spec, customer note, or prior discussion. The ledger should be sent soon after the meeting while memory is fresh. It should be short enough that people will actually read it. The goal is not perfect minutes. The goal is to prevent decisions from becoming vague memory.
