A meeting decision log captures why a team chose something, not just what the meeting discussed. This template helps teams preserve options, tradeoffs, owner, date, and revisit condition so the decision still makes sense weeks later. ## Why meeting notes are not enough Many meeting notes list topics and action items but lose the actual decision. A month later, the team remembers the outcome but not the rejected options or the constraint that mattered. This creates repeated debates because new teammates cannot see why the decision was reasonable at the time. ## Minimum fields Use seven fields: decision, date, owner, context, options considered, reason chosen, and revisit condition. The revisit condition is important because decisions age. A team might choose a manual process while volume is low, then revisit when requests exceed a weekly threshold. ## Keep the reason concrete Do not write "decided for simplicity" unless the note explains what simplicity meant. Better: "manual approval stays because request volume is under ten per week and automation would take longer than monthly review." This lets the team know what would change the decision later. ## Include rejected options briefly Rejected options do not need long essays. Name the option and the reason it lost. For example, "self-serve access portal: rejected for now because contractor roles still need manual review." This prevents the same option from being rediscovered as if nobody considered it. ## Add a visible owner The owner is not always the person doing the work. It is the person responsible for keeping the decision accurate. If the owner leaves or the process moves, the log should be reassigned. Otherwise the decision becomes an orphaned rule. ## Template - Decision - Date - Owner - Context - Options considered - Reason chosen - Revisit condition - Links or evidence This turns meetings from temporary conversation into a memory the team can inspect before reopening a debate.
