Config Change Incident Note

Config change incident note is a small incident artifact for failures caused by flags, environment variables, routing rules, limits, or external settings rather than application code.

Many incidents are not introduced by a code deploy. They happen when a feature flag is flipped, a rate limit is lowered, a queue concurrency setting changes, an API key scope is updated, a domain rule changes, or a third-party dashboard setting is edited. These changes can be harder to find because they may not appear in the commit history.

The note should capture who changed the setting, when it changed, which environment it affected, what value changed from and to, what dependent systems use it, and how the change was validated. Sensitive values should be redacted, but the shape of the change should remain visible. For example, “PAYMENT_WEBHOOK_SECRET rotated from version A to version B” is useful; the secret value itself is not.

The incident note should also include a blast radius question: which requests, users, regions, workers, scheduled jobs, or external callbacks could see this config? A flag used only on a preview route has a different risk profile from a shared auth secret or global cache setting.

The practical use is searchability. Six weeks later, a developer should be able to answer whether a strange failure matched a config change pattern without reading chat history. Configuration is part of the system, so its incident record should be as searchable as code review.
