Self-driving cars have killed people. Autonomous weapons systems have struck unintended targets. Warehouse robots have injured workers. The promise of autonomous systems has always come packaged with risk — and the engineering community is now producing the frameworks to manage that risk with more rigor than the first decade of widespread deployment produced. ## What "Safety" Means in Engineering Terms Safety is not the absence of failure. No engineering system operates without failure modes. **Safety** in the engineering sense means that system failures do not produce catastrophically unacceptable outcomes, and that the probability of such outcomes is quantifiably low. Autonomous systems introduce a specific challenge: their failure modes are not fully enumerable in advance. A conventional brake system can fail in understood ways. A neural network deciding whether an object is a pedestrian or a shadow can fail in ways that were never anticipated during design. > ⚡ The 2018 Uber ATG fatality investigation found that the system correctly identified the pedestrian but then discarded that classification — a failure mode that existed nowhere in the pre-deployment safety analysis. ## The Dominant Frameworks Three frameworks have emerged as industry standards: **1. ISO 26262 (Automotive Safety Integrity Level — ASIL)** Defines four safety integrity levels (A through D). ASIL-D represents the highest requirement — used for systems where failure could result in death. Requires systematic hazard analysis, fault-tolerant architectures, and independent verification. **2. NASA Systems Safety Handbook** Developed for spacecraft and aircraft, now applied broadly to complex autonomous systems. Key principle: failure modes and effects analysis (FMEA) must be exhaustively applied before deployment, with residual risk documented and accepted by responsible engineering authority. **3. DARPA's Assured Autonomy Program** Focuses on the core problem that traditional safety analysis cannot enumerate neural network failure modes. Output: formal verification methods — mathematical proofs that a controller will remain within defined operational bounds under specified conditions. ## Redundancy and Fallback Architecture The practical engineering response to unanalyzable failure modes is redundancy: - Waymo's vehicles use three independent sensor modalities (LiDAR, radar, cameras). Any two must agree before action is taken. - Flight control systems use triple-redundant computers with majority-voting — two computers must agree for any command to execute. - Nuclear reactor safety systems use N+2 redundancy with independent power supplies. The numbers are staggering when you calculate what full aerospace-grade redundancy would cost in a consumer vehicle. The industry is navigating a real tradeoff between acceptable safety margins and commercially viable price points. ## The Regulatory Gap The engineering frameworks exist. The regulatory frameworks are lagging. NHTSA's approach to autonomous vehicle certification remains non-prescriptive — manufacturers self-certify against their own safety cases. The EU's AI Act addresses high-risk systems but does not specify technical standards. ## The Bigger Picture Autonomous systems will continue expanding into more domains — surgical robotics, logistics, critical infrastructure, defense. The engineering tools to build safer systems exist and are improving. The institutional tools — regulatory bodies with technical expertise, liability frameworks that create proper incentives — are not keeping pace. Most coverage misses the point. Here's what's real: the governance problem is harder than the engineering problem. The engineering is worth understanding. The governance is worth demanding.
