A rollback note should preserve the next attempt

A deploy discussion note about recording rollback target, first symptom, evidence, and next safe attempt.

A rollback note is useful when it keeps evidence alive after the system is stable. I would include release identifier, rollback target, first symptom, affected path, and what still needs to be tested forward. “Rolled back” is recovery; it is not the same as root cause.
