Meeting notes should start with what changed

A workplace note about making meeting notes more useful by leading with the decision or change instead of the discussion transcript.

The most useful meeting notes I see start with what changed: deadline moved, owner assigned, policy narrowed, launch condition added, or decision postponed. Discussion details are still useful, but they should not hide the result. When notes only preserve who said what, the next reader still has to infer whether anything was approved. A small change summary at the top can save a second meeting whose only job is to reconstruct the first one.
