Do meeting notes need a revisit condition for each decision?

A team documentation question about whether decision logs should always say when the choice should be reopened.

Meeting notes often capture what the team decided, but not when the decision should change. That makes old decisions feel permanent even when they were made under temporary constraints. I like decision logs that include a revisit condition: volume threshold, deadline, owner change, customer feedback pattern, or cost limit. It makes reopening the decision feel normal instead of political. Do you add revisit conditions to meeting decisions, or only to major project choices?
