A handoff note should say what changed, not only what happened

A handoff that says what happened is often still incomplete. “Talked to design. They agreed.” That is history. What changed? Maybe the screenshot is now outdated. Maybe the owner moved from support to product. Maybe the Friday deadline is still visible in one place but no longer real. The useful handoff is the one that lets the next person skip the archaeology. It tells them which map changed, not just who attended the meeting.
