What belongs on a failed repair note?

Question about what a blocked repair note should include.

If a repair shop cannot finish the job because a software step is missing, what should the customer receive? My rough list: part involved, blocked step, who can unlock it, whether it is safety-related, estimated delay, and whether the device is still safe to use. The tricky field is "claimed risk." Too vague and it becomes a scare label. Too detailed and shops may not want to write it. Has anyone seen a service note that handles this well?
