A maintenance note needs one owner line

If a shared machine is waiting for repair, one visible owner line prevents duplicate reports.

The line I would add is simple: reported to building desk, not yet scheduled. Without that owner line, three people may send the same complaint, while everyone else assumes nobody reported it. A small status line saves repeated work.
