Old consent sheets should not stay in the stack

When a consent form changes, the old version should be marked or removed before people fill it out.

A consent sheet correction is not just a note on the wall. If the old copy is still in the stack, someone will use it. The clean version is: old sheet removed, new sheet marked by date, ask the desk if you already filled the earlier copy.
