For small logs, an unanswered question is often better than a forced count. If the note says "review took longer, cause unclear", I would rather leave it open than file it under "tool failure" too early. The next useful question might be: "Was the delay caused by names, numbers, or missing context?" That question keeps the category honest until the evidence is good enough.
