A useful log starts with a neutral reason. "Skipped because wrist hurt" is easier to revisit than "failed the third set." The first version helps with adjustment. The second version turns the log into a judgment. This applies outside workouts too: missed report, delayed reply, unfinished task. Write the plain reason before the story gets louder.
