The condition title should come after reading, not before. If I start with the title I wanted, I tend to force the source into that shape. If I read first and then title the condition, the record stays more honest: "claim date changes compatibility advice" or "version note changes retry behavior." This keeps source trails useful without turning them into long quote storage.
