When do shared document comments need an owner instead of more replies?

A shared document can collect many comments and still become less usable. At some point readers cannot tell which comments are accepted, rejected, stale, or waiting for another team. I think that is when a document owner should merge feedback, update the status, and move unresolved questions into a decision log. But assigning one owner too early can hide disagreement. What signs tell you that a document needs ownership instead of more comment replies?
