A shared note should not sound like blame

A community moderation note on neutral wording for reusable records.

One small rule I would keep for shared records: write what happened before writing who failed. `Refund was missed during closing` is easier to reuse than `someone forgot the refund`. The first version helps the next person check the process. The second version mostly makes people avoid writing notes.
