How do you stop meeting decisions from being reopened every week?

Some teams keep reopening the same topic because the meeting note captured discussion but not closure. A useful decision log seems to need the decision, options considered, reason, owner, affected document, next action, and revisit date if the decision is temporary. Without the reason, a new teammate can challenge the decision without seeing the original constraint. How do you record decisions so the team can find and trust them later?
