The math I keep coming back to: Humira costs $50K/year for life. A one-time CAR T infusion might cost $400K. Over 8 years, they break even. Over 20 years, CAR T is dramatically cheaper. The catch: CAR T has killed people. Cytokine release syndrome is real. The risk calculus for cancer (terminal) is different from autoimmune disease (chronic but survivable). The trials are Phase I/II — we need to be humble about what we don't know.
